We are committed to collecting, using and disclosing personal information responsibly and only to the extent necessary for the services we provide. Here are some of the main principles. We have a more detailed privacy policy, available at each of our six sites.

COLLECTION, USE AND SHARING OF PERSONAL HEALTH INFORMATION
Implied Consent
Our primary purpose in collecting, using and sharing your personal health information is to provide you with health care. We may also collect, use and share your personal health information in order to communicate or consult with other health care providers about your care. We share aspects of your personal health information to process payments through government programs, like the Ontario Health Insurance Plan (OHIP).

Express Consent
There are other circumstances where we are not allowed to assume we have your consent to release information. For example, we must have your permission to give your personal health information to people who do not provide you with health care, like your insurance company or your employer. We will also need your consent to communicate with any family members or friends with whom you would like us to give more detailed information about your health.

PROTECTING PERSONAL HEALTH INFORMATION
We understand the importance of protecting personal information. For that reason, we have taken several steps to safeguard the privacy of your information. This includes staff training in the collection, use and disclosure of information only as necessary to fulfill duties. Other steps to safeguard your privacy, including protection of electronic data, are outlined in the privacy policy.

YOU CAN LOOK AT YOUR INFORMATION
With only a few exceptions, you have the right to see what personal information we hold about you. Often all you have to do is ask your physician or health professional. We can help you identify what records we might have about you. We will also try to help you understand any information you do not understand (e.g., short forms, technical language). If you believe there is a mistake in the information, you have the right to ask for it to be corrected.
