CLINIC ON HOLD UNTIL FURTHER NOTICE

The On Track Diabetes Intensive Care Clinic Program is designed for patients with uncontrolled diabetes. Generally patients with an A1C greater than 0.09, frequent hypoglycemia or temporary initiation of prednisone are the criteria required to be referred to the clinic.

​

The goal of the program is to improve physical health and increase engagement of patients with complex diabetes identified with poor disease management using the Canadian Diabetes Association Guidelines.

​

The clinic is held twice a month on a Monday at 797 Princess St., Suite #312. Patients are seen by a Nurse Practitioner and may be referred to a Registered Dietitian if needed. Patients may be seen individually for follow ups depending on their needs. Initial appointments are 1 hour and follow up appointments vary from 15 minutes to 30 minutes.

Often insulin and/or oral anti-hyperglycemics are initiated and adjusted. Follow up is determined on an indivdual basis. 

Referrals to the program can be made by your Health Care Practitioner.
