Preventative Care |

Please contact your physician’s office if you require one of the following:
- Pap test – due every two to three years (35 yrs to 69 yrs)
- Mammogram – due every two years (50 yrs to 74 yrs)
- Childhood immunization – due at 2,4,6,12,15,18 months and 4-6 years of age
- Colon cancer screening – fecal occult blood testing due every two years (50 yrs to 74 yrs)
- Flu shots – please contact your physician’s office if you have received the flu shot elsewhere
If you have received a mail out from your physician regarding one of these preventative care categories and have any questions please contact your physician’s office.
